Global Medical Coverage	Reimbursed at 100% outside US, US territories and possessions and Canada
U.S. Medical Coverage	Reimbursed at 100% in-network and 80% out-of-network within the US, US territories and possessions and Canada
Medical Evacuation	Emergency Medical Transportation covered with pre-certification
Home Health Care	Provides 2 visits per day for maximum of 90 days
Deductible Options	$100, $250, $500, $1,000, $2,500 or $5,000
Prescription Drugs	Covered 100% after deductible – no annual maximum
Mail Order Pharmacy	Available for maintenance drugs (deductible is waived)
Wellness Exam	Routine Physical to $400 (deductible is waived)
Mammogram	Covered 100% once per year, within $400 annual amount for Routine Physical
Allergy Injections	Covered 100% after deductible
Maternity	Covered same as any illness.
Well-Baby Care	6 visits in the first 12 months
Bridge Coverage	Available for 3 months during job transition (group rates apply)
Continuance	Available for 18 months (individual rates apply)
Eligibility	No age limit for employees, dependents eligible to age 75
Dependent Children	Covered from birth to age 19 (23 if full-time student)
Mental Health Care	In Patient to a maximum of 30 days per year, Out Patient up to $2,500 per year with a $25,000 lifetime maximum
Acupuncture	Covered at 100% outside the U.S., treatment inside the U.S. subject to coinsurance (by licensed physician)
Chiropractic Care	Reimbursed at $30 per visit for a maximum of $1,000 per year
